All students in the Band Program must have a completed Emergency Medical Treatment Form on file. Do not leave any blank lines. Please use the word "none" for any lines that do not apply.
It is critical that we maintain current contact information with you regarding mailing address, phone number, and email. All students must fill this form out once a year.
Summer Band is a required course of all students who participate in the Freedom Band Program. All students receive a grade and 0.5 credits for Summer Band as part of Summer School. It is equivalent to the number of hours a student would spend in a class for an entire semester.
What to bring!
High school band programs strive to give students a life-long love for music and music-making. However, like any other worthwhile experience, a tremendous amount of cost goes toward operational needs. OCPS provides minimal support for day-to-day operations of the performing arts programs. To provide such support, each student is asked to contribute his/her fair share. Students may pay their fair share directly by writing a check OR students may fundraise their fair share. All individual student profit from fundraisers will go toward fair share.
Students cannot learn who are not in attendance at rehearsals and performances. All rehearsals and performances are mandatory and all absences are initially considered unexcused. However, unforeseen situations arrive, and an absence may be excused. NO ABSENCE WILL BE EXCUSED WHEN NOTIFICATION IS RECEIVED VIA EMAIL OR PHONE CALL. NO ABSENCE WILL BE EXCUSED WHEN NOTIFICATION IS RECEIVED MORE THAN ONE SCHOOL DAY AFTER THE ABSENCE. To apply for an excused absence, please fill out the form below.
